• Commercial General Liability – Occurrence Form
• Contractors Pollution Liability – Occurrence or Claims Made Form
• Microbiological Decontamination – Claims Made

Premium and Limits:

$1,000,000 (Higher limits available)
Minimum Deductible: $10,000
Minimum Premium: $5,000

Terms:

• Annual Policy Term.
• Coverage for microbiological decontamination activities
• Professional Liability available for indoor air quality assessment
• Retroactive date of policy inception unless evidence of prior coverage can be provided
• No surplus lines taxes

Submission Requirements:

• Legends Environmental Services application
• Legends Mold supplemental questionnaire
• Company Statement of Qualifications, including resumes of key personnel, a detailed description of past project experience, and training ..certificates for all company personnel
• Company written standard operating procedures (SOP), including an SOP for microbiological decontamination if those services are provided
• Standard company contracts. Contract must state that microbiological growth could reoccur if the source of the moisture is not remedied
• Past two years financial statements
• Past five years loss runs
• Company specific health and safety program
